Step 1: Get the following documents

For official proof and verification, we would require you to submit the following documents for the type of claim you are making.

 

Document Requirements for Hospital and Surgical Claim: 

  1. Hospitalisation & Surgical Claim - Claimant's Statement
  2. Hospitalisation & Surgical Claim - Attending Physician's Statement
  3. Outpatient Claim - Claimant's Statement
  4. Hospitalisation & Surgical Claim - Checklist for Individual Claim
  5. Hospitalisation & Surgical Claim - Direct Credit Facility Form

 

Step 2: Submit your documents

  1. i-Get In Touch (iGIT)
  2. Your Great Eastern Takaful Agent

 

Note: For reimbursement on hospital and surgical claims incurred above RM500, original documents are required before payment approval. Kindly submit the relevant documents to our Head Office for our processing.
