For official proof and verification, we would require you to submit the following documents for the type of claim you are making.
Once you have downloaded and completed the relevant claim forms, please submit the claim to us together with the required documents via i-Get In Touch (i-GIT) for assessment.

We understand that you are keen to get a fast reply from us. Please note that every online submissions can now be tracked by using our Track & Trace function in the i-GIT for faster and easier claim submissions status.
Alternatively, you may also submit all the claims documents personally to our Customer Service Centre located at level 3 at the head office or the nearest branch to you.
