Step 1: Get the following documents

For official proof and verification, we would require you to submit the following documents for the type of claim you are making.

1. Accident Claim Form - Claimant's Statement – to be completed by certificate owner / claimant
2. Accident Claim Form – attending physician’s statement
3. Letter of Authorisation/Consent
4. Family Claims -Document Checklist
5. Direct Credit Facility Form
